To me, this picture shows Grigori Rasputin, a devilish monk, taking control of the Russian Monarch. With Nicholas II off at the war front, and Alexandra in Russia leading the people, Rasputin thought it was a good time to step in and "guide" Alexandra in her doings to the country of Russia. The hand around Alexandra shows that Rasputin is trying to take her and mold her into what he wants and what may not be in the countries best interest. It looks as if Alexandra is looking up at Rasputin because she doesn't know what to do, which may incorporate his hand around her. Nicholas II turns his head as if he sees nothing is going on, which in reality he doesn't because he is at the war front with his troops and he has no idea what is going on in Russia at the time.
